Application Instructions



Thank you for considering applying to our graduate program in Mathematics. Please read all instructions thoroughly before starting this process. The seven steps below detail what you will need to do. Feel free to contact us if you have any questions not answered here. 

International and domestic students have some different requirements. These are very important to note. Each degree type has certain specific requirements to take note of as well.

Current and former Ohio State graduate students usually do not have to re-apply to Ohio State Graduate Admissions. You will instead need to submit your materials electronically directly to the Math Graduate Office. Please also complete the university's Transfer of Graduate Program form on GradForms


The Department of Mathematics PhD and MMS programs accept applications only for the Autumn semester of the upcoming academic year. The deadline for Regular applications is 15 December 2017. We will accept Standby applications through 29 January 2018. However, due to the volume of applications we receive and the time limits we have on making fellowship funding decisions, Standby applicants are reviewed on a space-available basis only. Documents such as transcripts, test scores (GRE and TOEFL / IELTS), and letters of recommendation can arrive beyond the deadline.  However, the graduate recruitment committee can only make decisions based upon complete applications. All documents are to be submitted electronically via the Graduate & Professional Admissions website. If you are interested in both the MMS and PhD programs, you may apply to both by submitting a single application for the PhD and declaring in your personal statement that you also wish to be considered for the MMS. 

The MAQRM (actuarial science) program accepts applications for both Autumn and Spring Semester admission. The international student application deadline for Spring Semester MAQRM admission is November 10. The domestic student application deadline for Spring Semester MAQRM admission is December 1. The application deadlines for Autumn Semester MAQRM admission are the same as those for our other graduate programs.

University Requirements

There are university-wide minimum requirements which are non-negotiable:

  • 4-year Bachelor's (or higher) degree from an accredited college or university. 3-year undergraduate degrees are not generally recognized by Ohio State as meeting this requirement*
  • Minimum Grade Point Average (GPA) in the 75th percentile of the grade scale. For example: the GPA has to be at least 3.0 on a standard 4.0 grade scale. Minima for some international grading systems may be different (e.g., second class/upper division in class system) - please inquire.
  • International students must provide proof of English proficiency within the last two years with one of the following:
    • Test of English as a Foreign Language (TOEFL) score of at least 79 for the internet-based test or 550 for the paper-based test
    • International English Language Testing Service (IELTS) exam with score of at least 7.0
    • Exempted are applicants who are citizens of or who have received a bachelor’s degree or higher from one of the following countries: Australia, Belize, the British Caribbean and British West Indies, Canada (except Quebec), England, Guyana, Ireland, Liberia, New Zealand, Scotland, the United States and Wales. US Permanent Resident Aliens (Green Card holders) are exempt from this requirement if they have been in the US for at least one year prior to their term of enrollment. Students from US territories such as Puerto Rico are NOT exempted unless they have received a prior degree from an institution in one of the 50 states.
    • Note: International MMS Applicants are expected to present a TOEFL Speak score of 28 in addition. (Successful PhD applicants typically have speak scores at or above 20 but occasionally we make admissions below this threshold as well).

*   This can be petitioned. A Master's degree or higher degree meets this requirement.

Step 1

The application process begins online at the Graduate Admissions website.  In the application, prospective students will be asked to provide:

  • Contact, demographic and citizenship information
  • Educational history
  • Test scores
  • Degree desired

Please enter your name exactly as it appears on the biographical information section of your passport. Failure to match that can cause the inability to obtain a US student visa.

Please note that if your last name is long (over 16 letters), and/or contains a hyphen, or there are multiple last names, we recommend that you ask us which way to enter it. 

If you are a domestic applicant and wish to be considered for underrepresented groups/minority-based fellowships, you will need to answer the Ethnicity portion of the application.

Step 2

  • The campus for all applicants is Columbus
  • When selecting the program, your choices are Mathematics - PHDMathematics - MMS, or AQRM – MAQRMOnly MMS has a sub-plan option to select from: Mathematical Biosciences, Computational Sciences, and Mathematics for Educators
  • Ultimate Degree Objective is optional
  • Minority students (US citizens only) are strongly encouraged to indicate their ethnicity and race. This information is REQUIRED in order to be eligible for possible Graduate Enrichment Fellowships which provide full funding without teaching obligations for the first year.
  • Your permanent mailing address is important as that is where all admissions documents will be sent if you are living in the US
  • The email address you enter is critically important, please ensure that it is correct and check it regularly

Step 3

The admissions site also provides an opportunity to request electronic references (letters of recommendation). You should have at least 3 recommenders.  In the application, prospective students will be asked to provide the recommenders':

  • Name
  • Email address
  • Institution address
  • Phone number (with country dialing codes)

Only if necessary, or if there is a problem, recommendations may be instead sent by email ( by using this PDF icon reference form [pdf] with the first page ONLY filled out by you, then sent to the recommender to fill out the second page and attach their letter. The recommender should send it directly to the math department.

After you submit an application, you may add as many references as you like.  Wait until after you submit and the application posts, and you receive your OSU name.# ID.  Go to to activate the username, then:

To Add a recommender:

  • Log into your Application References Summary using your Ohio State username and password.
  • Click the Add a Reference to an Application button.
  • Read the Application References - Instructions and click ACCEPT
  • Enter recommender name, organization phone, mailing address and email address on the Application References - Details page.
  • Select the application and term for which the recommender will direct their letter, and indicate whether or not you wish to Waive Access to view recommendation letters after they are submitted.
  • Check to make sure information is correct and click Save.
  • To add additional recommenders, click OK to access the Application References - Summary screen, then click Add a Reference to an Application.

Step 4

Applicants must attach scans of the following documents to their application. Please limit each to a maximum of 3 pages if possible.  

  • Statement of Purpose 
  • C.V. / Resume
  • List of Mathematics Courses (with Year TakenInstructor / Textbooks Used / Grade Received). Please use the following form for this: PDF icon Math_Courses_for_Applications_Fillable [pdf]
  • Other Relevant Documents (optional) Please do not send an entire thesis or a book, we will not read it. Limit to a maximum of no more than 10 pages.
  • International students are encouraged to provide a scan of the biometric information page of your Passport. This is not required, however, it does aid in clarifying names and reduces delays in admissions document processing if we admit you.

Step 5

Applicants are required to submit one academic transcript from all previous and/or currently-attended colleges/universities.  Applicants have the option to upload transcripts when submitting the online application, or via email at Graduate Admissions. The scans should...

  • be of actual transcripts, not advising reports or web printouts
  • be in color and legible: if we cannot read the document then we cannot evaluate your credentials for the program 
  • include the back information section of at least one page of the transcripts from each school

You may submit an updated version, if so desired, as newer grades become available. NOTE: If you list a school, we will need a transcript from that school. If your transcripts list another school (i.e. transfer credit situation) we will need a transcript also from that additional school.


Step 6

Educational Testing Services (ETS) scores should be sent directly by ETS to Ohio State Graduate Admissions. The Institutional code for Ohio State is 1592 (please do not use the major field code, leave it blank).

GRE Math Subject Test scores are required of all PhD applicants. This can be waived on rare occasions for significantly advanced students. Requests for waivers should be submitted through our waiver request web form with a brief explanation as instructed in the preamble of the form.

GRE General Test scores are required for all international students who are not exempt from the Ohio State English proficiency requirements. These are not required for domestic students. However, a score is required for all students who wish to be considered for fellowship competitions.

Step 7

Once all fields have been filled and documents attached, applicants will be prompted to submit payment. By selecting the Mathematics MMS or PhD program you will be eligible for the specially reduced fee of $5.00. MAQRM applicants must pay the regular OSU application fee.

Once the application is complete, students can view their application status on the Graduate Admissions site. The Mathematics Graduate Office has to process all of the attached documents individually, so please do not be concerned if the incomplete status remains on submitted materials until approximately February or early March.

You are always welcome to let us know that you would like to stop by and tour the department. When possible, we try to accommodate individual visits, however, please understand that this has no bearing upon our committee decisions. Students admitted for Autumn Semester will be invited to visit campus during Spring Semester in order to help make their decision by the national deadline of 15 April.
